Our services are provided by a caring and knowledgeable team of Registered and Licensed Practical Nurses, Certified Nursing Assistants, Medical Social Workers, and Physical, Occupational, and Speech Therapists. Our employees are bonded, insured, and background checked. A nurse is available on call 24 hours a day 7 days a week for problems that may arise after office hours. 

Each client will have a primary Registered Nurse (RN) who will follow them admission through discharge. At Home Health Care Plus, Inc. we feel this continuity of care is essential to effective communication and healing. Your nurse will see you each visit in your home and is available for questions about your care in person or via phone during your time with us.

What is Home Health Care and will my insurance pay for care in the home?

Home Health Care are services provided to patients in their own homes under the supervision of their physician. Many patients recover sooner and more comfortably in their own home so we coordinate with hospitals and nursing homes to help people return home as quickly as possible and resume their normal activities.
Services include:
  • Skilled Nursing Care (RN, LPN) – Wound Care, Lab Draws, Ostomy Care, Cardiac & Respiratory Assessments, Catheter Care, IV Therapy, Medication Teaching & Management
  • Certified Nursing Assistant (CNA) – Bathing & Personal Care, ROM Exercises, Ambulation & Transfer Assistance, Light Housekeeping, Meal Preparation, Respite Care & Errands
  • Therapy – Physical, Occupational, and Speech
  • Medical Social Worker – Evaluations
We will always coordinate our services with your insurance coverage. Medicare will pay 100% for qualified patients.

What private duty services do you offer for my loved one who has Alzheimer’s?

Our private duty agency specializes in the care of patients with Alzheimer’s disease and other forms of dementia. Our team of professionals offers home care to patients of all ages ranging from intermittent skilled visits to 24 hour care 7 days a week. Our services include, but are not limited to:
  • Skilled Nursing – lab draws, med box fills, wound care, ostomy care, catheter care, IV therapy.
  • Certified Nurse Aide – bathing and personal care, light housekeeping, meal preparation, companionship, medication reminders, respite care and errands.

I fell at home and fractured my hip. I have been in a skilled facility receiving therapy. When I return home can I continue Physical Therapy with Home Health Care?

Yes, you will be evaluated by a Physical or Occupational therapist to provide a safety assessment, strengthening and exercises to prevent further falls. A therapist will continue to visit you at home until you are able to go to Outpatient Therapy or you have reached your goals.

My spouse has a wound, and he is confined to a wheelchair. Can care be provided in the home?

Yes your physician can order Home Health Care with specific orders to treat the wound. A Nurse will assess and instruct in wound care and evaluate other needs present. All our nurses are trained in wound care. We also have a wound specialist on staff that can be consulted.

I live in Peoria County and was just diagnosed with COPD (chronic obstructive pulmonary disease). I have new medications and breathing treatments, can a nurse come to see me?

We serve 7 area counties, including Peoria, Tazewell, Woodford, Fulton, McLean, Mason and Logan. A home nurse can help you in setting up your medications in a pill box and instruct you in correctly using your nebulizer for breathing treatments.
